Supporting documents (optional)

Attach evidence of your affiliation, intended use or authorization. Documents support review; approval is still required.

For organizational requests, consider an endorsement letter, authorization letter or relevant agreement. Include only information needed to assess this request.

Verify your identity with Fayda before attaching PDFs. Reviewers will see who uploaded the documents.

Verification is valid for 15 minutes and one successful submission. It identifies the uploader; organizational authority remains subject to review.
